Thank you for registering your member in our Lean Programs and Events (hereafter “Lean Program”). Lean Sensei International (a division of iCreative Service Inc.) (“Lean Sensei”) looks forward to having your member join our Lean Programs. By registering individual(s) (“Participants”) for this program, your organization, and the individual(s) you register, are agreeing to the following terms and conditions.

During the course of Lean Sensei International’s (hearby called LSI) programs, either LSI staff or your program’s classmates may take photos and/or videos. These photos and/or videos are typically used at LSI’s blogs, Facebook page, and other social media sites.  The photos/videos may also be used in LSI’s promotional materials or websites.  We are careful to only use photos/videos where necessary and where these provide useful information for our clients.  However, to ensure that we do not post photos/videos of anyone who might be uncomfortable having their photos/videos shown in a public environment, please review the following photo/video waiver.  Please note that in most cases, the photos and/or videos taken during the programs are used primarily in our blog and website to show readers of our sites what kind of programs or activities are currently going on at LSI.

I hereby state and declare that I am of the age of majority.  On behalf of myself, and my successors, heirs, assigns and anyone else who may make any claim for or on my behalf, I hereby irrevocably and unconditionally:

1.      grant to LEAN SENSEI INTERNATIONAL the irrevocable and unencumbered right to use my photograph or video image (the “Attributes”) for publicity purposes, in any and all media, without compensation or further notification including, without limitation, the right to produce and reproduce any materials produced by or on behalf of LEAN SENSEI INTERNATIONAL incorporating the Attributes; and to communicate, publish or distribute all or any part of the Attributes and materials referred to in this paragraph, by any means and in any medium, whether now known or hereafter developed, throughout the world. I hereby waive moral rights with respect to any materials produced incorporating the Attributes. I acknowledge that I shall not have or claim to have any right, title or interest in or to any photograph or video materials produced hereunder incorporating the Attributes;

2.      This document is governed by the laws of the Province of British Columbia and operates to the benefit of the Released Parties, as well as their administrators, successors and assigns, and is binding on me and my heirs, administrators, successors, assigns, insurers and estate.
